The St. Cloud VA Health Care System seeks a Hematology/Oncology Physician Assistant to serve Veterans at Community Based Outpatient Clinics in Brainerd, Alexandria and Montevideo. This full-time role is based at the Alexandria, MN location with up to 75% travel among clinics.

You will perform chronic disease management, preventative care, and evaluation of episodic illness, collaborate with a multidisciplinary team, and provide patient education within a Whole Health framework.

Qualifications

  • Assess and document medical history, diagnosis, and plan of care.
  • Initiate, orders, and evaluate plans of care for outpatient clients.
  • Provide follow-up exams for medication management and monitoring.
  • Educate patients and families regarding disease process, risk factors, symptoms and treatment.

Responsibilities

  • Operate within the Hematology/Oncology scope as part of the Primary and Specialty Medicine Service Line.
  • Collaborate with a multidisciplinary team to deliver comprehensive Veterans healthcare.
  • Manage chronic diseases and preventative care for a predominantly male veteran population.
  • Maintain documentation of patient care plans and follow-up outcomes.

Summary

St. Cloud VA Health Care System is seeking a Physician Assistant - Hematology/Oncology to serve Veterans at our Community Based Outpatient Clinic (CBOC) locations - Brainerd - Alexandria and Montevideo. This position is full-time - based out of the Alexandria - MN location - with at least 75% travel between all the clinics. This vacancy is posted as a Physician - Physician Assistant - and Advanced Practice Nurse for one position.

  • Functions or Scope of Assigned Duties: Assesses and documents medical history - diagnosis - and plan of care
  • Initiates - orders - and evaluates plans of care for a panel of outpatient clients
  • Provides follow-up exams for mediation management and monitoring
  • Provides patient/family education regarding disease process - risk factor - symptom control - and treatment (including use of medication)
  • Consults with other providers and multidisciplinary team
