Orders diagnostic tests and interprets data, discriminating between normal/abnormal findings. Formulates, implements and evaluates an ongoing plan of care incorporating interdisciplinary input. Manages treatment modalities. Counsels and supports patients and families, utilizing appropriate knowledge, training and experience. Facilitates/provides opportunity for improvement in outcomes for service line patient population. Performs interventions per approved standardized procedures.* Documentation Creates/Maintains appropriate documentation and record keeping for all patient contacts.* Education Assists in developing learning experiences to meet the needs of staff, community, and patients.
