Job summary

LiveWell Homecare Agency seeks a licensed Physician Assistant to provide patient care in collaboration with physicians, exercising autonomy in evaluation, diagnosis, and treatment. You will order tests, interpret results, and participate in follow-up care in a home-based setting.

You will document in electronic medical records, educate patients, communicate with staff and physicians, and ensure safe, ethical care while meeting clinical goals and adherent to policies.

Qualifications

Job description

Role: Provides patient care in collaboration with physicians, demonstrating autonomy in evaluation, diagnosis, treatment, and follow-up. Orders and interprets tests, prescribes medications, and performs procedures within scope. Communicates with patients, staff, and physicians to ensure safe, ethical, and effective care, acting as a patient advocate and educator.

Responsibilities: Assess and diagnose patients, establish treatment plans, document accurately in electronic medical records, and adjust therapies as needed. Report significant findings to supervising physicians and perform procedures per license. Engage in telephone and in-person communication, follow policies, and meet clinical goals.

Qualifications & Skills: Bachelor’s or Master’s Degree in Physician Assistant studies, NCCPA certification or eligibility, Pennsylvania licensure, and CPR certification. Strong knowledge of care management, clinical policies, patient safety, and documentation. Excellent communication and clinical assessment skills.
