Chestnut Health in Illinois seeks a Physician Assistant to join our physiatry team, delivering patient-centered rehabilitation care in SNF settings. You will collaborate with therapists, nurses, and physicians to develop comprehensive planning and support functional independence throughout the care continuum.

Qualifications

Licensed to practice in the state of employment
Current Controlled Substance License (required on start date)
Current DEA License (required on start date)
NCCPA Certification
Interest in Physiatry/Physical Medicine and Rehabilitation
Orthopedic experience (nice to have)
Physiatry experience (nice to have)
Skilled Nursing experience (nice to have)

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
Conduct patient assessments including history, exams, and diagnostics to determine rehab goals.
Monitor progress and adjust treatment plans to improve functional outcomes and independence.
Participate in multidisciplinary meetings, case conferences, and educational activities to enhance clinical skills.
Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR systems.
Educate patients and families about rehabilitation processes, outcomes, and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
