Job summary

Post Acute seeks a Physician/Medical Director to provide clinical direction and oversee healthcare providers in post-acute facilities. You will deliver in-patient holistic medical services and guide care planning, evaluation, and treatment of chronic and acute illnesses.

You will collaborate with nursing and physician teams, ensure continuity of care, and contribute to quality improvement initiatives while remaining current with best practices in medicine.

Qualifications

  • Proven experience as a physician providing primary care in a post-acute setting.
  • Experience prescribing treatments and health regimens to treat resident/patients.
  • Experience collaborating with other health care professionals in diagnosis and treatment.

Responsibilities

  • Provide primary medical care within post-acute facilities.
  • Serve as resource for decisions, planning, and policies affecting health of residents/patients.
  • Exercise professional judgement and provide consultation to Providers on treatment.
  • Collaborate with Physician Assistants, Nurse Practitioners, and staff to identify resident/patient needs.
  • Monitor residents/patients in post-acute settings for continuity of care.
  • Assist in emergency treatments and complex procedures.
  • Adhere to policies, safety, and infection control standards.
  • Participate in health care committees (QAPI, Antibiotic Stewardship).
  • Foster a climate of trust and compassion for patients/residents.
  • Stay current with medical developments by attending conferences.

Job description

The Physician/Medical Directo r is responsible for clinical direction and oversight of H ealthcare P roviders in various post-acute facilities , providing in-patient holistic medical services utilizing innovat ive technologies to improve patient access impact ing quality outcomes. The se medical services include planning , directing , and evaluat ing resident/patient care ; examination and treatment of chronic and acute episo dic illnesses ; and treatment of minor injuries, accidents, and other injuries/illnesses. Performs elected laboratory testing , writ es prescriptions, and diagnos es and treat s medical conditions within the scope of the ir license expertise, protocols, and available equipment.

Duties and Responsibilities
  • Works with the Post Acute Medical T eam to provide patients with primary medical care services within the standard s of practice and care for facility patients/residents.
  • Acts as a resource person for decisions, planning, and policies affecting the health of residents/patients.
  • Exercises professional judgement and provides consul tation to Pr oviders concerning appropriate treatment of residents/patients.
  • Collaborates with P hysician A ssistants, N urse Pr actitioners , and facility staff to identify resident/patient needs and helps develop plans to meet these needs.
  • Follows residents/patients in a post -a cute skilled nursing facility , inpatient rehabilitation, assisted living , and other post-acute care facilities , as needed for continuity of care.
  • Assists P roviders with emergency treatments and more complex procedures.
  • Adheres to facility policies, procedures , and objectives ; on going quality improvement objectives and safety ; and environmental and infection control standards.
  • Serves as a member of committe es dealing with health care , to include Quality Assurance/Performance Improvement (QAPI) , and Antib iotic Stewardship, etc.
  • Cultivates a climate of trust and compassion for patients/residents.
  • Remain s up to date with developments and b est practices in medicine by attending confer ences and seminars.
Specific Requirements
  • Proven experience as a physician providing primary care and diagnostic services in a post-acute setting.
  • Experience prescribing appropriate medications, treatments, and health regimens to treat patient/resident health issues.
  • Experience collaborating with other health care professionals in treatment and diagnosis of patient/resident medical issues and conditions , on admission and through discharge planning, as appropriate.
