Job summary

Pc Cfl is seeking an experienced Family/Internal Medicine or General Practice physician to deliver comprehensive geriatric primary care in Central Florida. You will work with a physician-led team serving older adults in skilled facilities, assisted living, and homes, with an established patient base and referral network.

Qualifications

• Board-certified or board-eligible physician in Family Medicine, Internal Medicine, or General Practice.
• Active Florida medical license.
• Current DEA license.
• Proven clinical experience in geriatric care.

Responsibilities

• Evaluate, diagnose, and treat geriatric patients.
• Develop individualized treatment plans.
• Monitor and adjust treatments as needed.
• Coordinate care with specialists and facility teams.
• Educate patients and families on health maintenance and prevention.
• Maintain accurate medical records and facilitate transitions of care.
• Build strong relationships with patients and facility staff.

The Opportunity

As a physician on our team, you will provide comprehensive primary care to geriatric patients while developing meaningful, long-term relationships with patients, families, and facility teams.

You will care for patients with complex medical needs while working collaboratively with our clinical team to coordinate care and support the overall health and well-being of each patient.

Your responsibilities will include:

• Evaluating, diagnosing, and treating assigned patients
• Developing individualized treatment plans
• Monitoring patient conditions and adjusting treatment as needed
• Coordinating care with specialists and other healthcare professionals
• Educating patients and families regarding health maintenance and disease prevention
• Facilitating referrals and transitions of care
• Maintaining accurate and timely medical records
• Communicating effectively with patients, families, facility staff, and members of the care team
• Building strong relationships with patients and the facilities we serve

What We're Looking For

• Board-certified or board-eligible physician in Family Medicine, Internal Medicine, or General Practice
• Active, unrestricted Florida medical license
• Current DEA license
• Proven clinical experience
• Strong clinical judgment and problem-solving skills
• Ability to develop strong relationships with patients, families, and facility staff
• Familiarity with electronic medical records and standard medical technology
• Bilingual skills are a plus
