Job summary

Powerback Pediatrics in Maywood, IL, is seeking a dedicated School-Based Physical Therapist to support students aged 3–21 within the school environment.

You will implement measurable treatment plans, collaborate with a multidisciplinary team, and document progress for IEPs, attending related meetings and ensuring compliance with state guidelines.

Qualifications

• Must have a valid and unencumbered Physical Therapy license in the state of practice.
• Qualifying degree in Physical Therapy is required.

Responsibilities

• Implements treatment plans focused on functional outcomes tied to IEP goals.
• Collaborates with Teachers, SLP, OT, PCP, and others for cohesive care.
• Assesses and documents student progress with formal/informal measures.
• Prepares annual review reports and goals per IEP dates and state guidelines.
• Attends school-related meetings (IEP, annual reviews, domains, referrals).
• Conducts screenings/evaluations to determine PT requirements in school settings.

Position Summary

Our School Based Physical Therapists serve students with various physical and developmental challenges, ranging in age from 3-21 years old.

Responsibilities

• Implements treatment plans that focus on functional and measurable outcomes to be shared with the school and/or supervisors, as mandated in the student’s Individualized Education Plans (IEPs).
• Consults with other professionals (Teachers, SLP, OT, PCP, etc.) on the student's treatment team to ensure a cohesive and comprehensive therapy team.
• Assesses and documents each student’s progress through formal and informal tests and measurements.
• Writes and prepares annual review reports and goals for each student in accordance with their IEP dates and requirements, in order to remain in compliance with state guidelines.
• Attends all school related meetings as scheduled, such as, but not limited to, Triennials, Annual Reviews, Domains, Referrals, IEP, and pre-IEP meetings for each individual student.
• Provides screenings and evaluations to determine requirements for skilled physical therapy intervention relative to the student/client’s level of function within the school environment. This includes, but is not limited to, the following areas: school function, range of motion, strength, positioning, contracture management, and conducting home assessments.

Qualifications

• Must have a valid and unencumbered Physical Therapy license in the state(s) of practice, or proof of license and eligibility.
• Must have a qualifying degree in Physical Therapy.

Posted Salary Range

USD $44.00 - USD $47.00 /Hr.
