Hartford HealthCare Rehabilitation Network in Connecticut is seeking a licensed Physical Therapist to join our outpatient and acute care teams. You will assess patients’ physical function, design personalized treatment plans, and deliver a range of therapies to promote safe, effective recovery.
Collaborating with Rehab Therapists, Nurses, and Social Workers, you will help coordinate discharge planning and connect patients with community resources. New graduates are welcome with mentored support.

Collaborate for Continuity:Work closely with a multidisciplinary team—including Rehab Therapists, Nurses, and Social Workers—to ensure seamless care coordination. You’ll also play a key role in discharge planning and connecting patients with community resources to support their ongoing recovery.
Knowledge, Skills and Abilities:
Demonstrates strong time management skills and the ability to prioritize daily assignments effectively.
Able to read, understand, and follow oral and written instructions, and communicate information clearly.
Proficient in basic computer skills, including email and internet use. Strong working knowledge of Windows-based programs, including word processing, spreadsheets, databases, and presentation software.
Working knowledge of State Practice Act guidelines and Federal Medicaid and Medicare regulations.
Problem Solving and Decision Making
Exercises sound judgment and discretion while performing duties independently and consistently with minimal supervision.
Demonstrates strong analytical skills to gather and interpret information from multiple sources.
Responds appropriately to changing circumstances and resolves issues effectively.
