Meadowbrook Healthcare is seeking a Physical Therapist PT to join our team. You will screen patients, perform evaluations, and develop treatment and discharge plans, while documenting progress and supervising a Physical Therapy Assistant(s).

The role emphasizes implementing new techniques to enhance services, conducting patient and family education, and delivering high-quality physical therapy with patient-first care for long-term residents.

Qualifications

• Must hold a valid Physical Therapist license (PT).
• Strong in identifying patient needs in long-term residents.
• Ability to supervise Physical Therapy Assistant(s).

Responsibilities

• Screening patients.
• Performing evaluations and developing initial and interim treatment and discharge plans.
• Documenting progress toward identified objectives.
• Overseeing Physical Therapy Assistant(s).
