Job summary

Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. We offer a comprehensive total compensation package and a supportive, family-like culture that fosters professional growth.

Licensed PTAs will provide evidence-based, skilled therapy under supervision, with opportunities for mentorship, CEUs, licensure reimbursement, and specialty certification paid for by CRG.

Qualifications

• Licensed Physical Therapy Assistant required.
• Open to therapists at all experience levels.
• Must be eligible to practice as a PTA.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under supervision of a Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient progress and communicates with the supervising Physical Therapist to adjust treatments.
• Provides consultation and counseling to patients, families, caregivers and other service providers related to physical disorders.
• Contributes to discharge summaries of services provided per regulatory and clinical requirements.

• Provides skilled therapy services to patients who need rehabilitation services.

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
