Physical Therapy Assistant (PTA) - Home Health Far West Suburbs

Position Overview

Location: Far Western Suburbs including Aurora, Montgomery, Oswego, Plainfield, Bollingbrook, and surrounding communities.

Hours: Days.

Key Responsibilities
  1. Patient Relationship: Develop and maintain restorative relationships throughout the healthcare continuum, following Advocate's MVP of Compassion. Incorporate an individualized approach to the plan of care, assess patient and family readiness, and promote knowledge and confidence in self-care and infection prevention.
  2. Providing and Improving Quality Care: Ensure delivery of quality, cost-effective care, adapt practices based on evidence-based standards, and promote patient safety and risk reduction. Document thoroughly to support reimbursement and regulatory compliance.
  3. Professional Development: Participate in staff meetings, educational activities, and utilize resources to stay current. Demonstrate effective communication and conflict resolution skills, and seek opportunities for self-improvement.
  4. Care and Service Coordination: Work effectively within the care team, make sound clinical decisions, recognize clinical changes, and support discharge planning and interdisciplinary communication.
  5. Business Development and Growth: Adhere to policies, support care plans, and monitor resource utilization to optimize outcomes.
  6. Patient Education: Assess learning needs, modify approaches as necessary, and utilize appropriate educational materials to enhance patient and family understanding.
Licensure and Experience

Licensed in Illinois or eligible; demonstrate compliance with state continuing education requirements. Minimum 2 years of clinical experience or strong clinical affiliation experience.

Knowledge, Skills & Abilities

Proficiency in computer use, professional communication, documentation, and use of therapy equipment. Must hold Illinois State License; certification required within 6 months if not already certified.

Physical Requirements & Working Conditions

Extensive travel within the geographic area, ability to lift up to 35 pounds, and work in diverse environments including patient homes and assisted living facilities. Must be able to perform physical tasks such as bending, standing, transferring patients, and climbing stairs.
