Job summary

PruittHealth Home Health is seeking a licensed Physical Therapist Assistant to provide in-home physical therapy services under the supervision of a physical therapist. The role supports patients in their residences with a focus on effective plan-of-care implementation and collaboration with the care team.

The PTA will contribute to a patient-centered home health program, maintain documentation and licenses, and participate in regulatory surveys.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• Foreign-trained graduates must have credentials deemed substantially equivalent by a credentials evaluation organization approved by the APTA.

Responsibilities

• Assist with the development and implementation of the plan of care within goals defined by the physician, patient/patient family and interdisciplinary team.
• Collaborate with the interdisciplinary team to develop, integrate, and manage the patient care plan of care.
• Demonstrate intermediate knowledge of home care and competency in discipline for specific patient care skills.
• Meet agency productivity standards and utilize time and resources effectively and efficiently.
• Complete documentation timely, accurately, and at the point of care, according to industry standards.
• Assist the physical therapist in assessing the physical disability and level of function of patients.
• Maintain current license, certification, and clinical records to meet federal, state and agency regulations and guidelines.
• Participate in center/agency surveys (Licensure / JCAHO) and any required reports.
• Report alleged or suspected violations to supervisor and/or others until resolved.
• Be self-directed, prioritize, manage time, and meet outcomes and deadlines.

JOB PURPOSE:

Provide physical therapy treatments to PruittHealth Home Health patients in their place of residence under the supervision/direction of a physical therapist.

KEY RESPONSIBILITIES:

• Meets agency productivity standards and utilizes time and resources effectively and efficiently.
• Completes documentation timely, accurately, and at the point of care, according to industry standards.
• Assist the physical therapist in assessing the physical disability and level of function of patients.
• Maintains current license, certification, and clinical records to meet federal, state and agency regulations and guidelines
• Participate in center/agency surveys (Licensure / JCAHO), and any subsequently required reports.
• Accepts responsibility to report any alleged or suspected violations to supervisor and/or others until the issue is resolved.
• Ability to be self-directed, prioritize, manage time appropriately, and meet outcomes and strategic goals by targeted deadlines.

Qualifications

LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS:

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist Assistant entry level education in the U.S. by a credentials evaluation organization approved by the APTA.

REQUIRED SKILLS AND EXPERIENCE:

• Minimum of 2 years' therapy experience.
• Valid driver’s license and automobile liability insurance.

PREFERRED SKILLS AND EXPERIENCE:

• Prior Home Health experience is preferable.
