Job summary

PruittHealth is seeking a qualified Physical Therapy Assistant to provide in-home physical therapy to patients under the supervision of a licensed physical therapist. You will help implement the plan of care, document care at the point of service, and support ongoing patient progression in a home health setting.

The role requires PTA program completion, active state licensure, and a valid driver’s license with auto-liability coverage.

Qualifications

• Must have a valid, unrestricted professional license in the state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• Foreign-trained applicants must have substantially equivalent PTA education as determined by a credentialing body approved by the APTA.

Responsibilities

• Assist with development and implementation of the plan of care within defined goals.
• Collaborate with the interdisciplinary team to manage and integrate patient care.
• Assess patient function and assist in developing the care plan under supervision of a PT.
• Meet productivity standards and document timely at the point of care.
• Maintain licensure, certifications, and clinical records according to regulations.

Job Purpose

Provide physical therapy treatments to PruittHealth Home Health patients in their place of residence under the supervision of a physical therapist.

Key Responsibilities

• Assist with the development and implementation of the plan of care within goals defined by the physician, patient/family and interdisciplinary team.
• Help maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan.
• Demonstrate intermediate knowledge of home care and competence in discipline-specific patient care skills required for the provision of care.
• Meet agency productivity standards and utilize time and resources effectively and efficiently.
• Complete documentation timely, accurately, and at the point of care, in accordance with industry standards.
• Assist the physical therapist in assessing the physical disability and level of function of patients.
• Maintain current license, certification, and clinical records to meet federal, state and agency regulations and guidelines.
• Participate in center/agency surveys (Licensure / JCAHO) and any subsequently required reports.
• Report any alleged or suspected violations to supervisor and/or others until the issue is resolved.
• Be self‑directed, prioritize, manage time appropriately, and meet outcomes and strategic goals by targeted deadlines.

Licensure, Certification and Education Requirements

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• If foreign‑trained, the program must be deemed substantially equivalent to a Physical Therapist Assistant entry‑level education in the U.S. by a credentials evaluation organization approved by the APTA.

Required Skills and Experience

• Minimum of 2 years’ therapy experience.
• Valid driver’s license and automobile liability insurance.

Preferred Skills and Experience

• Prior Home Health experience is preferable.
