Reedy River Post Acute in Greenville, SC is looking for a compassionate Physical Therapist Assistant to join our rehabilitation team. Working under a licensed Physical Therapist, you will provide skilled services to help residents improve mobility, strength, and independence.

You will document sessions, collaborate with nursing and therapy staff, assist with discharge planning, and participate in care conferences.

Qualifikationen

• Graduate of an accredited Physical Therapist Assistant program.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• SC PTA license in good standing with CE requirements.

• Provide skilled therapy services under supervision of a licensed Physical Therapist to help residents improve mobility, strength, function and independence.
• Implement individualized treatment plans according to physician orders and therapy goals.
• Monitor resident progress and communicate updates to the Physical Therapist.
• Document treatment sessions, resident responses and progress notes accurately and timely.
• Collaborate with nursing staff and interdisciplinary team to support positive outcomes.
• Assist with discharge planning and caregiver education.
• Participate in care conferences and team meetings as needed.
• Maintain a safe, clean, and organized therapy environment; ensure compliance with regulations.
