Legacy Healthcare Services, Inc. is seeking a qualified Physical Therapy Assistant (PTA) for a PRN hybrid role, combining in-facility care at TerraBella Thornblade with personalized home therapy through our Better at Home program.

You will work with senior adults in their homes and residential communities across Greenville County, delivering 1-on-1, functional-based sessions and building individualized care plans while maintaining autonomy in treatment.

Formación

• Current license to practice PTA in state of practice.
• Excellent interpersonal and clinical skills.
• Computer proficiency and comfort with documentation/records.

Responsabilidades

• Provide comprehensive 1-on-1 patient care per physician plans and state Act.
• Deliver rehabilitation services both in-home and at home-base facility.
• Assess home environments, mobility, gait, ROM, and tailor rehab plans.
• Educate patients and caregivers on exercises, safety, and home programs.
• Collaborate with interdisciplinary team and participate in care planning and referrals.
• Document progress summaries and supervise therapy assistants per state regulations.

Coverage: In-Home Care for Greenville County

Schedule: PRN | 10+ hours dedicated to TerraBella Greenville

Additional hours supporting TerraBella Thornblade, TerraBella Summit, and Springs at Simpsonville

Legacy Healthcare Services is searching for an experienced, motivated, and compassionate Physical Therapy Assistant (PTA) to join our team. This unique hybrid position combines in-facility care at a home base facility with personalized home therapy through our Better at Home program.

You will work directly with senior adults both in their residential community and in the comfort of their homes; empowering them to lead lives filled with purpose, safety, and joy. With private, functional-based, 1-on-1 treatment sessions, you will have the autonomy to individualize care plans and truly build meaningful relationships with your patients.

Key Responsibilities

• Comprehensive Patient Care: Evaluate, assess, and provide Choose an item. services in accordance with physician plans of care and the State Practice Act.
• Dual-Setting Treatment: Deliver high-quality, 1-on-1 rehabilitation services both on-site at your home base facility and directly in patients' homes throughout the county.
• Environment & Mobility Assessment: Evaluate home environments, mobility, gait, muscle strength, and ROM potential to build tailored rehabilitation programs.
• Caregiver Education: Instruct patients, family members, and caregivers on therapeutic appliances, home exercises, and safe mobility strategies.
• Interdisciplinary Collaboration: Participate in clinical conferences and coordinate care plans as part of a multi-disciplinary rehabilitation team. Identify additional patient needs (such as PCA services) and make appropriate referrals.
• Documentation & Supervision: Prepare timely progress summaries and supervise therapy assistants in accordance with state regulations and company policy.

Position requirements:

• Associate’s of applied science degree in area of discipline
• Current license in state of practice
• Excellent interpersonal, fiscal, and clinical skills
• Computer proficiency

Pay Range

USD $28.00 - USD $38.00 /Hr.
