Job summary

AccentCare Inc. is seeking a dedicated Physical Therapist Assistant to join our Fall River home health team.

You will work under a licensed Physical Therapist, delivering treatment, educating patients and families, and documenting care within agency policies. Responsibilities include coordinating with the supervising therapist, managing a caseload, and participating in team conferences to ensure quality patient outcomes.

Qualifications

• Must hold an Associate degree from an accredited Physical Therapy program.
• Must possess and maintain valid CPR certification.
• Must have a valid driver’s license and be able to travel to all locations.
• Ability to educate patients and families on home safety and medical equipment use.

Responsibilities

• Provide patient care and document treatments per physician orders and agency policy.
• Document interventions, treatment priorities and patient progress.
• Educate and counsel patients and families; communicate findings to supervising therapist.
• Present patient information to the supervising therapist and team; participate in care planning.
• Deliver skilled PT interventions within agency productivity standards; collaborate with team.

What You Need to Know

The Physical Therapy Assistant, under the supervision of a licensed Physical Therapist, is responsible for providing treatment to patients, functioning as a member of the interdisciplinary team, provides patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation according to policies and procedures and current standards of practice. The Physical Therapist Assistant manages a caseload in conjunction with supervising therapist and in accordance with agency productivity standards,

attends and participates in patient and other conferences and agency in-services as assigned.

If you meet these qualifications, we would love to meet you:

• Associates degree from an accredited school of Physical Therapy
• Have a valid license, registration, or permit to practice in state of agency operation
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations
• Be able to implement care plans and communicate any findings or changes in regards to patients back to the Physical Therapist and physician
• Ability to educate patients and their families in basic home safety and medical equipment safety

Responsibilities

As a Physical Therapy Assistant, you will:

• Provides patient care and documents treatment in accordance with physicians orders, agency policy and patient needs.
• Documents appropriate interventions, approaches, strategies, materials and establishing treatment priorities.
• Provides organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to supervising therapist and keeps team informed of patient status.
• Provides appropriate skilled treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Seeks and accepts assistance from team members in treatment planning and intervention.
• Accurately documents, completes and submits daily notes, assists with other required forms in a timely manner to prevent interruptions in patient care.
• Meets agency productivity standards and uses work hours efficiently.
• Attends and participates in agency meetings and committees as assigned.
• Appropriately negotiates the patient' s participation in treatment, treatment times and treatment plans.
