Job summary

Saber Healthcare Group is seeking a Physical Therapy Assistant for a skilled nursing facility serving Autumn Care of Altavista and Forest Health & Rehab (Lynchburg, VA). The role is full-time with day shifts, on-site at the Altavista/Forest Health facilities. Wages range from $28.00 to $35.00 per hour, with comprehensive benefits.

You will assess residents, implement therapy plans, and educate on maintaining function post-discharge, within a supportive, team-oriented environment.

Qualifications

• Must have an Associate’s Degree in a Physical Therapy Assistant program.
• Must possess a current, unencumbered license to practice as a Physical Therapy Assistant within the state.
• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.

Responsibilities

• Return residents to optimal physical function and assist in achieving patient and therapy goals in a SNF setting.
• Instruct residents in maintained function post-discharge through education and independent exercise.
• Support activity modification and education for ongoing recovery after therapy.

Position Summary:
