You will assist in implementing treatment plans, document progress in the EMR, and participate in weekly interdisciplinary meetings, ensuring high quality, patient-centered care.

The role includes educating staff and caregivers, coordinating program development, and maintaining licensure requirements.

Qualifications

• Graduate of an accredited therapy program.

Responsibilities

• Plan, implement, or administer programs that restore or enhance performance in individuals with functional impairments under the Physical Therapist.
• Assist Physical Therapist with implementing treatment plans.
• Report patients' progress and discharge recommendations to supervisors.
• Attend and report on a patient’s status at weekly UR meetings and interdisciplinary care plan meetings.
• Observe and record patients’ progress, response to treatment and limitations in the EMR.
• Complete required documentation at the point of service.
• Identify need for modification of the treatment plan and report to the Physical Therapist.
• Educate staff, family and caregivers as indicated.
• Identify needs for program development and implement programs according to policies and procedures.

Job Summary

Physical Therapy Assistants work under a physical therapistto help our residents with physical injuries or illness of to regain rangemovement and control pain. Under the PT’s direction and plan of care, the PTAhelps residents who have lost motor function due to accidents, illness or age.

Responsibilities and Duties

• Works under the direction of thePhysical Therapist to plan, implement, or administer programs that restore orenhance performance in individuals with functional impairments.
