Post-Acute Rehab Solutions Therapy is seeking a dedicated Physical Therapy Assistant for PRN home health visits in Dallas, TX. You will work under a licensed PT to provide quality, personalized care, develop treatment plans, and document progress in patients' homes.

Responsibilities include meeting patient goals, promoting mobility, and collaborating with the healthcare team to optimize outcomes. Prior experience in home health or rehab is preferred.

Qualifications

• CPR certification required for patient care; maintain up-to-date training.
• Proven work experience as a Physical Therapist Assistant in home health or rehab settings preferred.
• Professional liability insurance is required to practice independently in home health settings.

Responsibilities

• Identify and meet patients' goals and needs.
• Provide PTA services to patients in the home setting under supervision of a licensed PT.
• Develop care plans using various treatment techniques and monitoring progress.
• Create fitness- and wellness-oriented programs tailored to patients' needs.
• Document patient care services and communicate progress to the care team.
• Collaborate with other health professionals to optimize patient outcomes.
• Motivate patients during treatment to improve strength, flexibility, balance and coordination.

Responsibilities

• Identify and meet patients' goals and needs.

• Provides PT assigned services to patients in the home setting in accordance with Physician plan of care and under the supervision of a licensed PT.

• Offer cost-effective treatments that help improve clients' motion and mobility.

• Reduce the need for medications and provide alternatives to surgery.

• Develop care plans using a variety of treatment techniques.

• Create fitness- and wellness-oriented programs tailored to patients' specific needs.

• Performs miscellaneous job-related duties as assigned by Supervisor.

• Provide quality, personalized and evidence-based care and proven interventions.

Requirements

• PTA Certification for the state of TX
• CPR Certification

• Multi-disciplinary collaboration for patient care

• Ability to manage patients with different types of personalities.

• Current knowledge of treatment practices
