Job summary

CARE FIRST HOME HEALTH CARE INC is seeking a Physical Therapy Assistant for home health services. The PTA provides services under the direction of the Physical Therapist and follows the physician's plan of care.

The role includes guiding patients through therapeutic exercises, using modalities, and documenting progress while ensuring safety and compliance. A licensed PTA with CPR certification is required, along with reliable transportation for visits.

Qualifications

• Must graduate from an accredited physical therapy assistant program.
• Current certification or license as required by state law.
• Demonstrates good verbal and written communication and organization skills.
• Must possess and maintain current CPR certification.
• Must be a licensed driver with a reliable vehicle and insurance as required by the organization.

Responsibilities

• Provide direct physical therapy under the direction of a physical therapist and physician's plan of care.
• Treat patients to relieve pain, develop or restore function, and maintain maximum performance.
• Instruct patients in active and passive therapeutic exercises, gait and functional training, and prosthetic training.
• Utilize physical agents (e.g., heat, cold, water, light, electricity, ultrasound, massage).
• Observe and report patient responses and condition changes to the PT and organization staff.
• Instruct patients in use of wheelchairs, braces, crutches, canes, and prosthetic/orthotic devices.
• Maintain clinical notes and records within required timeframes.
• Participate in Home Care Team meetings and inservice education.
• Perform other duties as assigned.

Job Description Summary

Gives Report To: Physical Therapist or Rehab Supervisor The physical therapy assistant provides physical therapy services under the direction of the physical therapist and according to the physician's plan of care.

Roles & Responsibilities

• Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care.
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance.
• Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.
• Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.
• Observes and reports to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s condition.
• Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
• Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record in prescribed time frames.
• Participates in Organization Home Care Team Members meetings and Organization Inservice Education Program.
• Performs other duties as assigned.
