Job summary

Covenant HealthCare seeks a Physical Therapy Assistant to support the Physical Therapist’s plan of care, delivering therapy and teaching patient home exercise programs. The role emphasizes compassionate care aligned with Covenant’s Extraordinary Care values.

Qualifications

• Graduate of a two-year college level program approved by the American Physical Therapy Association.
• At least one year of experience as a Physical Therapist Assistant
• BLS certified or eligible
• Homecare knowledge preferred
• Ability to type 25 WPM
• Demonstrate critical thinking skills
• Comfortable with basic computer programs and e-mail
• Strong interpersonal skills
• Good organizational skills
• Self-motivated

Responsibilities

• Physical therapy activities planned, delegated and supervised by the physical therapist.
• Discusses established discharge goals during treatment and provides information on goals met/unmet to physical therapist.
• Educates patient and family/caregiver per prescribed physician’s plan of care.
• Instructs patients in care and use of equipment as directed by physician and physical therapist.
• Maintains interdisciplinary communication with other disciplines via written and verbal means.
• Completes required agency forms to record time and patient progress in a timely manner.
• Assists with other clinical record keeping as deemed appropriate.
• Participates in inservice programs and staff meetings as required.
