Job summary

HCR Home Care in Batavia, NY is seeking a qualified Physical Therapy Assistant to provide patient care under the supervision of a licensed Physical Therapist. The PTA delivers services prescribed by the attending physician and assists in developing patient care plans.

Key duties include treating patients to relieve pain, instructing on use of prosthetic/orthotic devices, documenting progress notes, and collaborating with the care team to support functional goals.

Qualifications

  • Graduate of an accredited Physical Therapy Assistant program.
  • Current, valid PTA license issued by NYS Department of Education required.
  • One year of practical experience as a licensed Physical Therapy Assistant.

Responsibilities

  • Treat patients to relieve pain and/or develop or restore function to reach the highest level of independence possible.
  • Instruct patients and their families in the use of prosthetic, orthotic and assistive devices (canes, walkers, wheelchairs, etc.).
  • Write clinical progress notes on the date of the visit for inclusion into the patient’s permanent record.
  • Attend Physical Therapy staff meetings, in-service presentations and case conferences.
  • Perform an on-site joint visit with the assigned Physical Therapist initially, and communicate regularly for instruction in changes in the patient’s program.
  • Communicate regularly with the case manager and other health care professionals assigned to a case.
  • Instruct patients and their families in transfer techniques, home exercise program and activities of daily living as assigned by the physical therapist.
  • Other duties as assigned.

Education

Graduate of an accredited Physical Therapy Assistant program
NYS PTA license
One year PTA experience
