A home health care agency is seeking a licensed Physical Therapy Assistant to deliver in-home therapy. • Collaborate regularly with the supervising PT and interdisciplinary care team.

Schedule: Part-Time with weekend rotation

What You’ll Do

• Implement personalized treatment plans developed by a licensed Physical Therapist, helping patients regain strength, range of motion, and independence
• Deliver hands‑on care including therapeutic exercises, mobility training, gait education, and adaptive strategies to support recovery
• Document all interventions and patient responses in EMR to track clinical progress
• Educate patients and caregivers on home exercise routines and fall prevention strategies, empowering them with confidence
• Collaborate regularly with the supervising PT and interdisciplinary care team to ensure holistic patient care
