Job summary

Pathways Healthcare is seeking a qualified Physical Therapist to provide in-home therapy to patients under physician orders and state guidelines. The role includes evaluating patients, designing individualized rehab plans, and coordinating with caregivers to ensure continuity of care.

Qualifications

• Must hold a degree as a Physical Therapist approved by an accredited organization.
• Minimum of two years of relevant experience as a Physical Therapist Assistant, preferably with home health experience.
• Demonstrate excellent verbal and written communication, and strong organizational abilities.
• Maintain current BLS CPR certification.
• Must possess a valid driver’s license and own reliable, insured vehicle.

Responsibilities

• Provide physical therapy services per physician’s plan of care and state Physical Therapy Practice Act.
• Assess functional status, home environment, mobility, gait, and ROM to determine rehab potential.
• Develop and implement individualized PT programs and train involved family or caregivers.
• Participate in discharge planning and refer to other home health services as needed.
• Prepare and submit clinical progress summaries per policy guidelines.
• Provide PT assistant consultation to families and participate in peer consultations.

Position Details:

• Main Office Location: Stoneham, MA
• Service Area: Watertown, Waltham, Belmont Arlington, Lexington, Burlington areas.
• Job Type: Full Time
• Schedule: Monday - Friday

Responsibilities:

• Provide physical therapy services to patients in accordance with a written physician's plan of care and the state Physical Therapy Practice Act. This includes assessing and evaluating therapeutic/rehabilitative/functional status, evaluating the home environment, and determining the potential for rehabilitation, including muscle strength, mobility, gait, and range of motion.
• Initiate and develop individualized physical therapy programs for patients while also instructing other personnel and/or family/caregiver members involved in the patient's care. Actively participate in case conferences to ensure a comprehensive and coordinated treatment approach.
• Instruct patients and their families/caregivers in the use and care of therapeutic appliances and, when necessary, train patients in the use of prosthetic devices. Additionally, provide in-service education programs for nursing organization personnel as needed.
• Participate in discharge planning for patients, identifying their ongoing needs and referring them to other home health services as required.
• Prepare and submit clinical progress summaries based on the achievement of goals, following the guidelines set forth by the Organization policy.
• Offer physical therapy assistant consultation to home families/caregivers when indicated, and actively engage in the peer consultation process to enhance the overall quality of care.

Qualifications:

• Possess a degree as a physical therapist approved by an accredited organization.
• A minimum of two years of relevant experience as a physical therapist assistant, with preference given to those with community/home health experience is desirable.
• Demonstrate excellent verbal and written communication skills, as well as organizational abilities.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
