Job description

Shadelands Post Acute Rehabilitation Center is seeking a dedicated and passionate Physical Therapist Assistant to join our team on a full-time basis. Our facility focuses on delivering the highest quality care to our patients, with a current patient base that includes 59 Medicare patients and no long-term care residents.

As a member of our rehab team, you'll be part of a collaborative environment where we strive for excellence in patient care. Our team of therapists is highly supportive of one another, and we believe in working together to achieve the best possible outcomes for our patients.

Key Responsibilities:

• Treat patients as directed by Physical Therapist.
• Record daily treatment notes and weekly progress notes per PT Board.
• Assist in maintaining department. Participate in Patient Care and Rehabilitation Conferences, as needed.
• Assist with cleaning and maintenance of treatment area.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in in services training program for other staff in the facility. Record treatment changes per policy and procedures.

Qualifications:

• Active physical therapist assistant license
