Job summary

AdventHealth Altamonte Springs is seeking a dedicated Physical Therapy Assistant to join our Home Health team. You will work under the supervision of a Physical Therapist to support patient recovery, facilitate discharge planning, and help patients achieve their goals in their homes.

The role requires an Associate degree as PTA, BLS, a valid driver's license, and comfort with Microsoft Office. Prior home health experience is preferred. Full-time schedule, day shift, Altamonte Springs, FL.

Qualifications

• Associate degree in Physical Therapy Assistant required.
• Licensed PTA with CPR/BLS, valid driver's license.
• Minimum 1+ year home health experience preferred.

Responsibilities

• Maintain communication with the case manager, primary therapist, and other healthcare professionals to facilitate interdisciplinary continuity of care and appropriate discharge planning.
• Maintain productivity standards and adjust work hours to meet the needs of patients.
• Participate in departmental meetings, education sessions, and performance improvement initiatives to maintain clinical competencies and adhere to professional and evidence-based practices.
• Monitor patient response to treatment and communicates with the Physical Therapist for necessary adjustments.
• Educates patients, family members, and caregivers on identified needs, ensuring understanding. Implements treatment plans established by an Physical Therapist utilizing recommended techniques, adhering to insurance guidelines and patient needs.
• Documents interventions and patient progress accurately, completely, and timely, using measurable and objective terminology in line with standards of practice, insurance guidelines, and department/hospital policies and procedures.
• Other duties as assigned.

Job Description
