Job summary

Freedom Home Health and Hospice Care Services Inc. is seeking a Licensed Physical Therapy Assistant to deliver direct physical therapy under the supervision of a physical therapist in accordance with the physician's plan of care.

The role emphasizes treating pain, restoring function, and maintaining maximum performance through therapeutic exercises and patient training. It includes documenting care, utilizing modalities, and collaborating with the Home Care Team.

Qualifications

• Graduate of an accredited PTA program with an Associate’s degree or applied science approved by the American Physical Therapy Association.
• Current certification or license as required by state law.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with an insured automobile in good working order.

Responsibilities

• Provides direct physical therapy under the direction of a physical therapist and per physician's plan of care.
• Treats patients to relieve pain, develop or restore function, and maintain maximum performance.
• Directs and aids patients in therapeutic exercises, gait and functional training, and related activities.
• Utilizes physical agents (heat, cold, water, light, electricity, ultrasound, massage).
• Observes and reports patient reactions and changes to the physical therapist and organization staff.
• Instructs patients in care and use of mobility aids and prosthetic/orthotic devices.
• Maintains clinical notes and records within prescribed time frames.
• Participates in Home Care Team Meetings and Inservice Education Program.
• Performs other duties as assigned.

Job description

The physical therapy assistant provides physical therapy services under the direction of the physical therapist and according to the physician's plan of care.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care.

2. Treats patient to relieve pain, develop or restore function, and maintain maximum performance.

3. Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.

4. Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.

5. Observes and reports to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s condition.

6. Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

7. Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record in prescribed time frames.

8. Participates in Organization Home Care Team Members meetings and Organization Inservice Education Program.

9. Performs other duties as assigned.

POSITION QUALIFICATIONS
