Job summary

Visiting Nurse Home Care & Hospice seeks a dedicated Physical Therapist Assistant to support patients at home, implementing therapy plans created by a supervising PT. You will help restore mobility, improve independence, and enhance quality of life in patients receiving home care or hospice services.

This field-based role requires strong communication, reliable transportation, and the ability to work independently with an interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active PTA license in NH.
• Valid driver’s license and reliable transportation for home visits.
• Experience in home health, outpatient, rehab, or hospice preferred.
• Strong communication, observation, and patient teaching skills.

Responsibilities

• Provide therapeutic interventions—deliver exercises, gait training, strengthening, balance activities, and mobility support per the PT’s plan of care.
• Monitor patient progress—observe, document, and report changes in condition and functional ability.
• Educate patients and caregivers—teach safe mobility techniques and home exercise programs.
• Support hospice goals—focus on comfort, positioning, and quality of life interventions for hospice patients.
• Collaborate with the interdisciplinary team—communicate with PTs, nurses, aides, and social workers for coordinated care.
• Maintain accurate documentation—complete timely visit notes in the EMR.
• Ensure safety in the home—identify environmental risks and recommend modifications.

About the role

The Physical Therapist Assistant supports patients in their homes by implementing individualized therapy plans created by the supervising Physical Therapist. This role focuses on restoring mobility, improving functional independence, reducing pain, and enhancing quality of life for patients receiving home care services or hospice care.

You’ll work oneonone with patients, helping them stay safe, comfortable, and as independent as possible in their preferred environment.

What You’ll Do

• Provide therapeutic interventions — Deliver exercises, gait training, strengthening, balance activities, and mobility support per the PT’s plan of care.
• Monitor patient progress — Observe, document, and report changes in condition, functional ability, and response to treatment.
• Educate patients & caregivers — Teach safe mobility techniques, fallprevention strategies, and home exercise programs.
• Support hospice goals — Focus on comfort, positioning, gentle mobility, and qualityoflife interventions for hospice patients.
• Collaborate with the interdisciplinary team — Communicate with PTs, nurses, aides, and social workers to ensure coordinated care.
• Maintain accurate documentation — Complete timely, compliant visit notes in the EMR.
• Ensure safety in the home — Identify environmental risks and recommend modifications to support mobility and independence.
• Improve or maintain patient mobility and functional independence
• Reduce fall risk and promote safety in the home
• Support comfort and quality of life for hospice patients
• Provide compassionate, evidencebased care aligned with patient goals
