Job summary

Clinton Care Center is seeking a Physical Therapy Assistant to support licensed physical therapists in delivering personalized care for residents. You will assist with treatment plans, supervise therapeutic exercises, and ensure accurate documentation of progress and discharge summaries.

You will collaborate with the care team to promote holistic health, participate in team meetings, and pursue ongoing education to enhance your skills and knowledge in rehabilitation services.

• Assist physical therapists in the development and implementation of treatment plans for residents

• Conduct physical therapy treatments under the supervision of the physical therapist, including exercises, activities, and other interventions

• Monitor resident progress and report any changes or concerns to the physical therapist

• Maintain accurate records of resident treatment plans, including progress notes, treatment plans, and discharge summaries

• Participate in interdisciplinary team meetings to discuss resident care and goals

• Collaborate with other healthcare professionals to provide holistic care that meets the unique needs of each resident
