Job summary

Your Health is seeking a Primary Care Physical Therapy Assistant to join our team serving homebound patients across South Carolina, Georgia, and Florida. You will work with licensed therapists to implement treatment plans, supervise therapeutic exercises, and educate patients for optimal recovery in homes, ALFs, and ILFs.

The role is full-time and salary-based with a monthly bonus opportunity. Typical hours are Monday–Friday, 8:00 AM–5:00 PM, delivering compassionate rehab in community settings.

Qualifications

• Associate degree from an accredited Physical Therapy Assistant program.
• Current licensure or eligibility for licensure as a Physical Therapy Assistant in the state of practice.
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work effectively as part of a multidisciplinary team.
• Excellent organizational and time management skills.
• Compassion and empathy for patients undergoing rehabilitation.
• Physical stamina and dexterity necessary to assist patients with mobility and perform therapeutic activities.
• Pelvic Health Experience and/or interest.

Responsibilities

• Assist in implementing treatment plans and conducting exercises with patients.
• Educate patients on proper rehabilitation techniques and document progress.
• Develop individualized treatment plans and instruct patients in therapeutic exercises.
• Monitor patients during therapy sessions and provide guidance as needed.

What Are We Looking For?

Your Health is currently looking for a Physical Therapy Assistant to join our growing primary care family. As a Physical Therapy Assistant, you will work closely with licensed physical therapists to provide rehabilitative services to patients suffering from physical injuries or disabilities. Your primary responsibilities will include assisting in the implementation of treatment plans, conducting exercises with patients, educating patients on proper techniques for rehabilitation exercises, and documenting patient progress. As well as developing individualized treatment plans, instruct and assist patients in performing therapeutic exercises and activities, monitor patients during therapy sessions, providing guidance and support as needed. Maintain a clean and organized therapy environment, ensuring that equipment is properly sanitized and maintained. Communicate regularly with physical therapists and other healthcare professionals to coordinate patient care and adjust treatment plans as needed. Assist in the implementation of discharge plans, including providing patients with instructions for continued care and recommending appropriate follow-up services.

• Prior experience working as a Physical Therapy Assistant.

• Proficiency in electronic medical records (EMR) systems.
• Bilingual proficiency in English and another language commonly spoken by patients in the practice area.
