Job summary

Apex Rehab Group in Ulster County, NY is hiring licensed Physical Therapists to join our in-home care team. You can work part-time, full-time, or per diem, with scheduling that fits around your life while delivering patient-centered therapy at home.

The role focuses on creating personalized treatment plans, tracking patient progress, educating families, and documenting care with a supportive clinical team. A valid NY license and reliable transportation are required.

Qualifications

• Licensed PT in New York.
• Home health experience is a plus.
• Reliable transportation and valid NY driver's license.
• Strong communication skills and a positive attitude.
• Ability to manage your own schedule.
