Qualifications

• State licensure as a Physical Therapist (PT).
• Certified BLS or obtain it within 90 days of employment.

Responsibilities

• Perform initial and ongoing patient assessments.
• Promote continuous quality improvement.
• Teach and counsel patients and families.
• Set goals and develop treatment plans.
• Collaborate to identify and solve patient- and facility-wide needs.
• Participate in discharge planning including coordination and education.
