Job summary

Jackson Therapy Partners seeks a traveling Physical Therapist to support rehabilitation and chronic condition management in Perrysburg, OH, within a Skilled Nursing Facility.

Required: PT degree (BS/MS/DPt) from an APTA-approved program, active state license, and BLS certification if needed. NetHealth/PCC EMR experience preferred; 8:00 AM–4:30 PM, days, with a ½ lunch; 5 days/wk caseload plus 3 days.

Qualifications

• Active State License is required to start the assignment.
• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA.
• BLS Certification May Be Required from AHA or ARC.

Responsibilities

• Help patients recover from injuries and manage chronic conditions.
• Caseload: 5 times a week and 3 times a week, 7-8 hours daily.
• Provide therapy services in a Skilled Nursing Facility.

• Facility Type: Skilled Nursing Facility
• Schedule: 8:00 AM - 4:30 PM (flexible) with a ½ lunch (not paid)
• Shift: Days
• Patient Population: Rehab patients with various health conditions, orthopedic, patients of various ages, and a small dementia unit with orthopedic and deconditioning needs
• Caseload: 5 times a week and 3 times a week, 7-8 hours daily
• Experience: Preferred as this person needs to hit the ground running
• EMR: NetHealth/PCC
