Qualifications

• Provide comprehensive and compassionate therapy in a sub-acute/skilled nursing facility setting.
• Work collaboratively with other rehab therapists and nursing staff.
• Possess a current Physical Therapy license or be license eligible to practice in the state employed.
• Demonstrate strong computer skills with Microsoft Outlook, Word, Excel and experience with electronic documentation.

Responsibilities

• Deliver physical therapy services to residents in a sub-acute/skilled nursing setting.
• Collaborate with the rehab team to coordinate care and discharge planning.
• Maintain timely and accurate documentation of patient progress and treatments.
