Physical Therapist Position

Location: Stockton and All Surrounding Areas.

Compensation: Hourly and Paid Per Visit (PPV) hybrid model (negotiable based on experience, skill level, availability). Opportunity to accumulate overtime.

Essential Job Functions / Responsibilities

• Provides physical therapy services to patients according to a written physician’s plan of care and as defined in the state Physical Therapy Practice Act.
• Assesses and evaluates therapeutic, rehabilitative, and functional status, and participates in developing the total plan of care, including evaluating home environment.
• Evaluates muscle strength, mobility, gait, ROM, and potential for rehabilitation.
• Directs physical therapy treatment.
• Instructs patients and families/caregivers in the use and care of therapeutic appliances.
• Determines priority needs for physical therapy.
• Reports to the physician about patient’s reaction to treatment or changes in condition.
• Initiates the physical therapy program and instructs other personnel or family/caregiver members in certain phases of therapy, and participates in case conferences.
• May train patients in the use of prosthetic devices.
• Identifies patient and family/caregiver needs for other home health services and refers as necessary.
• Prepares and submits clinical and progress summaries based on attainment of goals.
• Participates in discharge planning for patients.
• Provides physical therapy consultation to home families/caregivers when indicated.
• Provides in‑service education programs for nursing organization personnel as needed.
• Participates in the peer consultation process.
• Supervises physical therapy assistants according to organization policy and state regulations.
• When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two weeks.

Position Qualifications

• Possesses a degree from a master’s or doctoral program in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist within the state, if applicable.
• One year of appropriate experience as a physical therapist; community/home health experience preferred.
• Demonstrates good verbal and written communication and organizational skills.
• Possesses and maintains current CPR certification.
• Licensed driver with a vehicle that is in good working order and insured in accordance with organization requirements.
