Job summary

Concept Rehab is seeking a full-time Physical Therapist (PT) in Linden, MI to join our rehabilitation team. You will evaluate patients, develop individualized treatment plans, and collaborate with nurses, OTs, SLPs, and the broader interdisciplinary team to restore function and independence.

Qualifications

• Current, unrestricted Physical Therapist (PT) license in Michigan.
• Graduate of an accredited Physical Therapy program.
• Strong communication, organizational, and clinical skills.
• Ability to work effectively with patients, families, and an interdisciplinary team.
• SNF or geriatric rehabilitation experience preferred.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve strength, mobility, balance, endurance, and functional independence.
• Monitor patient progress and adjust treatment plans as needed.
• Document evaluations, treatment sessions, progress notes, and discharge recommendations accurately.
• Collaborate with nursing staff, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team.
• Educate patients, families, and caregivers on exercises, safety, and functional mobility.
• Maintain compliance with applicable clinical, regulatory, and documentation requirements.
