Job summary

Qualifications

• Valid state license to practice as a Physical Therapist.
• Minimum of 1 year of experience in a SNF setting preferred.
• Strong clinical skills and knowledge of therapeutic techniques and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient-centered approach to care.
• Basic computer skills and ability to learn and use electronic medical record systems.

Responsibilities

• Conduct comprehensive evaluations to assess patients' physical abilities and limitations.
• Develop and implement individualized treatment plans based on patients' needs and goals.
• Utilize a variety of therapeutic techniques and modalities to improve patients' strength, mobility, balance, and overall physical function.
• Collaborate with other healthcare professionals to ensure coordinated and effective care for patients.
• Educate patients and their families on proper techniques for performing exercises and activities at home.
• Document patient progress and maintain accurate records in accordance with facility and regulatory guidelines.
• Participate in interdisciplinary team meetings and contribute to patient care planning and discharge planning.
• Stay up-to-date on current trends and best practices in physical therapy through continuing education and professional development opportunities.
