We are seeking dependable, experienced therapists who can deliver high-quality patient care, adapt to new facility environments, and maintain accurate documentation in an EMR system. A reliable PT license and transportation are required.

Qualifications

• Minimum 1 year of experience working under an active PT license.
• Experience working in skilled nursing facilities preferred.
• Must have reliable transportation to assigned facilities.

Responsibilities

• Utilize EMR to record patients’ medical information and vital signs.
• Evaluate residents' physical function, strength, balance, mobility, and rehabilitation needs.
• Develop and implement individualized treatment plans to improve mobility and functional independence.
• Provide therapeutic exercises, gait training, balance training, and other skilled physical therapy interventions.
• Educate residents, families, and caregivers on exercises, mobility techniques, and fall prevention strategies.
• Accurately document evaluations, treatments, and resident outcomes.

Responsibilities

An PT working contract through Elite Medical Staffing at a skilled nursing facility may be expected to:

• Utilize EMR to record patients’ medical information and vital signs
• Evaluate residents' physical function, strength, balance, mobility, and rehabilitation needs
• Develop and implement individualized treatment plans to improve mobility and functional independence
• Provide therapeutic exercises, gait training, balance training, and other skilled physical therapy interventions
• Recommend and train residents in the use of mobility aids, assistive devices, and other equipment as appropriate
• Monitor resident progress and adjust treatment plans based on clinical findings and goals
• Educate residents, families, and caregivers on exercises, mobility techniques, and fall prevention strategies
• Accurately document evaluations, treatments, and resident outcomes
• Maintain compliance with facility policies, regulatory requirements, and professional practice standards
• Communicate concerns or issues to onsite supervisor and agency
• Other duties as assigned
