Job summary

Gilroy Healthcare Center, a 24-hour skilled nursing facility in Gilroy, CA, seeks a Physical Therapist (PT) to evaluate residents, design individualized plans, and deliver evidence-based therapy to improve mobility and function. Collaboration with the rehab team and physicians is essential.

New graduates are welcome; licensure in California is required. The role offers Full-Time, Part-Time, and PRN options with a competitive hourly rate and a $4,000 hiring/retention bonus for eligible full-time

Qualifications

• Active California Physical Therapist license in good standing.
• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy.
• New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferred.
• Knowledge of PDPM, Medicare, and skilled nursing facility regulations.
• Strong leadership, communication, and organizational skills.
• Commitment to resident-centered, high-quality care.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Implement evidence-based interventions to improve functional outcomes.
• Document resident progress accurately and in compliance with regulatory requirements.
• Collaborate with interdisciplinary teams and physicians.
• Supervise Physical Therapist Assistants and therapy aides, as applicable.
• Monitor clinical outcomes and contribute to departmental performance.
