Job summary

HiredFirst is seeking a Physical Therapist (PT) for a 13-week travel contract in San Jose, CA. This role offers day shifts and 36-40 hours per week within a Skilled Nursing Facility, with competitive weekly pay and a chance to impact patient care.

Active California PT license or eligibility is required, SNF experience preferred, and new graduates with SNF rotations are welcome. BLS from AHA or ability to obtain upon offer is required.

Qualifications

• Active California Physical Therapist license (or eligibility).
• SNF experience preferred.
• New graduates with SNF clinical rotations welcome.
• BLS certification from the American Heart Association (or ability to obtain upon offer).

Responsibilities

• Evaluate patients and develop individualized plans of care using strong clinical reasoning.
• Deliver age-appropriate therapeutic interventions.
• Modify treatment plans based on patient progress and clinical findings.
• Document evaluations, treatments, and patient progress according to facility guidelines.
• Discharge patients when goals are met or services are no longer medically necessary.
• Collaborate with interdisciplinary team members to ensure high-quality patient outcomes.

Location: San Jose, CA (Not the right fit? We have additional PT opportunities available throughout California.)

Pay: $2,755 per week

Contract Length: 13 weeks (extension possible)

Schedule: Day shifts | 36-40 hours per week
