MARIONVALLEY in is seeking a licensed Physical Therapist to evaluate and treat patients, communicate with families, physicians, and the health team, and maintain documentation of services in medical records.

The role includes supervising PT assistants, developing treatment plans, and participating in discharge planning. A sign-on bonus is offered and familiarity with Casamba/Point Click Care is preferred.

Qualifications

• Bachelor's degree in Physical Therapy required; Masters/Doctorate preferred.
• Licensed Physical Therapist in the state of Ohio.
• Proficient in computer systems and electronic medical records.

Responsabilités

• Evaluate patients within 24 hours of referral or one business day; document delays with reason.
• Develop and obtain approval for treatment plans from referring physician.
• Treat patients per physician plan and document progress accordingly.
• Supervise PT assistants and students under state guidelines.
• Communicate patient progress with supervisor and health team members.
• Record evaluations, treatment notes, and progress notes in medical records.
• Instruct families and staff on maintenance programs and caregiver training for discharge.
