Job summary

Essentia Health in Minnesota seeks a licensed Physical Therapist to evaluate, plan, treat, and implement care for patients per APTA standards and Essentia policies. You’ll develop evidence-based treatment plans, educate patients, and coordinate discharge planning for a full caseload.

This 0.6 flex role offers 24–40 hours weekly with a $5,000 sign-on bonus, opportunities across outpatient, inpatient, and home health settings, plus a focus on professional growth and development.

Qualifications

• Bachelor’s, master’s, or doctoral degree in Physical Therapy from an accredited program.
• Active Physical Therapist license in the state of service.
• Willingness to work across outpatient, inpatient, and home health settings.

Responsibilities

• Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, educate patients and caregivers, and discharge planning for a full patient caseload.
• Manage documentation, billing, scheduling, and other administrative duties for a full caseload.
• Demonstrate professional communication and supervise support staff (licensed assistants, aides, volunteers).
• Collaborate with a multidisciplinary care team to optimize clinical and financial outcomes.
• Work with Essentia Health leadership and referring physicians to ensure cohesive patient care.
• May serve as a clinical instructor and participate in department or therapy discipline committees.

Education Qualifications:

• Bachelor’s, master’s, or doctoral degree in Physical Therapy from a physical therapy accredited program

We're offering a $5,000 sign on bonus for this position.

Key Responsibilities:

• Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, provide patient/caregiver education, and discharge planning for a full patient caseload.

• Provide all ancillary/administrative requirements for a full caseload of patients, including documentation, billing, scheduling management, and other administrative duties

• Demonstratesappropriate communication,professionalismand supervision of support staff (licensed assistants, aides, volunteers)

• Collaborate with a multidisciplinary care team to ensureoptimalclinical and financial outcomes.

• Work closely with Essentia Healthleadershipand coordinate with referring physicians to ensure cohesive patient care.

• Work where the patient need is highest (including flexing to other departments) when home department schedule allows

Rural Healthcare:

• Provide patient care across multiple settings, which may include outpatient, inpatient, and home health facilities.

• Deliver high-quality care to a diverse patient population with varying needs and conditions.

This position is a 0.6 Flex role. Minimum scheduled hours will be 24 hours per week, with the opportunity to flex up to 32-40 hours per week, depending on need.

Licensure/Certification Qualifications:

• Current Physical Therapist license in the state where services are performed
