Job summary

Lifepoint Health is seeking a Physical Therapist for ProHealth Rehabilitation Hospital of Wisconsin. The role is 32 hours per week (Thursday–Sunday), focusing on high-quality patient care and collaborative discharge planning.

You will assess, treat, and educate patients to restore strength, mobility, and function, while documenting outcomes and participating in team-based rehab care. A current PT license and CPR certification are required.

Qualifications

• Education: Graduate of an accredited physical therapy program.
• Experience: Previous experience in rehab setting preferred.
• License: Current PT license.
• Certifications: CPR certification required.

Responsibilities

• Conducts physical therapy assessments and develops treatment plans.
• Provides direct patient care to restore strength, mobility, and function.
• Educates patients and caregivers on home exercise programs.
• Documents treatment sessions and patient outcomes accurately.
• Collaborates with rehab teams and participates in discharge planning.
• Performs other duties as assigned.
