You will evaluate mobility and pain, document outcomes, and educate patients and families on home programs, while maintaining clinic readiness and safety standards.

Qualifications

• Licensed physical therapist with active state license and CPR certification.
• Minimum 2 years of clinical physical therapy experience in rehab settings.
• Proficiency in evaluating mobility, pain, and functional goals with measurable milestones.
• Ability to create individualized rehab plans and adjust based on progress data.
• Strong documentation skills with electronic medical records and outcome tracking.
• Experience in patient and family education on home programs and precautions.
• Collaboration with multidisciplinary team and care coordination for optimal outcomes.
• Performance-driven mindset with track record of meeting or exceeding targets.
