Job summary

Homeside Rehab is seeking compassionate Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide in-home, evidence-based physical therapy services in New York City and Yonkers, NY.

Qualifications

• Active New York State Physical Therapist (PT) or Physical Therapist Assistant (PTA) license required.
• Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or SNF preferred.
• Excellent clinical assessment and communication skills.
• Ability to work independently in a home health setting.
• Basic computer proficiency for electronic documentation.
• Valid New York State driver's license and reliable transportation.

Responsibilities

• Perform comprehensive physical therapy evaluations (PTs) and develop individualized treatment plans based on each patient's needs and goals.
• Provide evidence-based physical therapy interventions to improve strength, balance, coordination, mobility, gait, endurance, and functional independence.
• Deliver skilled therapy services in patients' homes while promoting improved function and quality of life.
• Complete accurate, timely, and compliant electronic documentation.
• Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care.
• Educate patients and caregivers on home exercise programs, mobility strategies, and therapeutic techniques.
• Communicate effectively with office staff regarding scheduling, patient updates, and clinical needs.

Homeside Rehab is seeking compassionate and dedicated Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide individualized, evidence-based physical therapy services to patients in their homes throughout New York City and Yonkers, NY.

Position Details

• Job Types: Full-time, Part-time, Per Diem

• Compensation: $75.00–$90.00 per hour

Schedule

• Choose your own hours

• Monday through Friday

• Day and evening shifts available

• Weekend availability optional
