InHome Therapy is seeking a licensed home care Physical Therapist (PT) to serve adult and geriatric patients in their homes around Oak Park, IL. If you value a flexible schedule, steady caseload, and a supportive team, we'd love to connect.

You will provide in-home PT services, assess mobility, develop goals, and educate patients and families while maintaining Medicare-compliant documentation. Mileage reimbursement and training are included.

Formación

• Active PT license in good standing.
• BLS CPR certification.
• Experience with in-home evaluations and progression of mobility goals.
• Comfort with EMR systems and digital documentation.

Responsabilidades

• Deliver in-home physical therapy to adult and geriatric patients in the community.
• Evaluate mobility, strength, and function to set individualized goals.
• Implement rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and prevention.
• Maintain Medicare-compliant documentation and timely records.

Compensation and Benefits*

• Mileage reimbursement – plus tools to reduce windshield time
• Paid training and mentorship
• The evaluation visit rate for this role ranges from $65- $85 per visit

Key Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in your community
• Evaluate patient mobility, strength, and function to develop individualized treatment goals
• Implement effective rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate and timely home health care documentation per Medicare and agency standards

Requirements

• Active physical therapy (PT) license in good standing
• BLS CPR certification
• Reliable transportation to see patients in their homes
• Basic comfort with technology - email, texting, EMR systems (training provided)

*This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO. Additional offerings may include 401(k), life and disability coverage, voluntary supplemental insurance options, and a company-paid Employee Assistance Program (EAP).
