Job summary

Concierge Home Care in Florida is seeking a Home Health Physical Therapist to deliver skilled therapy in patients' homes, aiming to improve mobility, strength and independence.

You will evaluate patients, develop plans, guide exercises, educate families, collaborate with physicians, supervise PTAs, and document visits using AI-assisted tools; a flexible schedule, onboarding, and ongoing training support your professional growth.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver's license with reliable transportation and auto insurance.

Your Role as a Home Health Physical Therapist

As a Home Health Physical Therapist , you will provide skilled therapy services in the comfort of patients’ homes while helping them regain mobility, strength, and independence.
