Zusammenfassung

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes, evaluating mobility, developing care plans, and guiding recovery in the home setting.

You will work autonomously with a supportive care team, supervise PTAs per state guidelines, educate patients and families, document progress in the EMR, and participate in team meetings.

Qualifikationen

• Graduate from accredited physical therapy program with DPT or Master’s degree.
• Current PT license in the practicing state.
• Minimum 1–2 years of home health experience preferred.
• CPR certification.

• Provide hands-on therapy to improve mobility, strength, balance, and independence.

• Support safe transitions of care and contribute to discharge planning.
• Collaborate with nurses, therapists, and physicians for coordinated care.

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
