Location: Oak Village Healthcare – Oaktown, IN

Schedule: PRN (As Needed)

Pay Rate: Up to $65/hr (5-10 hrs C-PRN Weekly*)

Revital Rehab Services is seeking a licensed Physical Therapist (PT) to provide PRN therapeutic care at our partner facility, Oak Village Healthcare in Oaktown, IN. This flexible role is perfect for therapists who want meaningful patient interaction while maintaining schedule flexibility.

Responsibilities:

• Perform evaluations and develop individualized physical therapy treatment plans
• Deliver evidence-based, patient-centered interventions
• Document care accurately and update goals as needed
• Collaborate with the interdisciplinary care team to support resident outcomes
• Maintain compliance with Revital’s quality, documentation, and ethical standards
