Physical Therapist /PT – Outpatient - HYBRID ROLE

Hybrid Role: Treating at the location 20 hours a week and 20 hours a week, Telehealth from home.

Location: Eagle Crest South, Lacrosse, WI

Setting: Assisted Living, Independent Living, Outpatient

Schedule: Flexible, up to 40 hours per week, Monday to Friday, No Weekends, No Evenings

Qualifications

• Current license as Physical Therapist or ability to obtain in the state of practice.
